POLICY ON RESPONSIBLE LENDING

To start the review process, you must submit the following documents:

  • Photo ID
  • Current bill - Must be an actual billing statement showing your full name, physical address, and current billing date (billing date must be within 30 days)
  • Two most recent pay stubs.
  • Copy of your Debit Card (front and back) - We do not accept credit cards.
  • Bank proof - Submit one of the following: Voided Check, Bank statement (Must show your full name and full account number), or a Bank Letter (Must be printed on bank letterhead stating your full name, routing number, full account number, and be signed by a bank representative).
  • Payroll Deduction Authorization (PDA) Form - You will need to download this form, fill out Employee Information, ID and Password, then sign and date at the bottom. Click here

All documents can be submitted by email to apply@superiorfunding.net or fax to 866-205-7605. Make sure to include your name and contact number when you submit your documents. All documents will be subject to review by a Superior Funding representative and must pass verification to determine approval.
